Required Documentation for Personal Exemption Applicants

  • If you are filing for Clause 41C (the $500 exemption) for persons aged 70 years and over, you will need to include copies of the following:
    • Verification of previous year's income
      1. Form SSA 1099, Social Security Benefits Statement
      2. W2 forms for all wages earned
      3. Any other Form 1099 showing interest, dividends, or other income
    • Verification of assets as of July 1st (beginning of current fiscal year)
      • Bank Statements for savings, checking, IRA's, and any other accounts
      • Most recent Real Estate Tax bill for property owned outside of Deerfield
    • Federal and State Income Tax Returns (if filed).
  • If you are filing for Clause 17D (the $175 exemption) for widows/widowers, persons over age seventy, and minors, you will need to include copies of the following:
    • Verification of assets as of July 1st (beginning of current fiscal year)
      • Bank Statements for savings, checking, IRA's, and any other accounts
      • Most recent Real Estate Tax bill for property owned outside of Deerfield
    • Federal and State Income Tax Returns (if filed).
  • If you are filing under Clause 22 for Disabled Veterans and:
    • You are filing for the first time; you will need a copy of your DD214 and a statement from the VA verifying your disability.
    • Our disability rating is 100%; you will need an updated statement from the VA verifying your disability.
    • Your disability is 10% or greater but less than 100%, verification is required for the first year application only.
  • If you are filing under Clause 37 for the Legally Blind, you will need to furnish an updated copy of your Certificate of Blindness from the Commonwealth.
